Payments

 

Billing and Insurance Information: The client is responsible for payments of all services rendered unless otherwise arranged with the therapist.  I occasionally work with clients on a sliding scale when space permits. 

 

Often insurance agencies will reimburse you for my services even if I am not on your insurance panel.  If I am an "out of network provider" you will need to pay for services at the time of service.  You are responsible for obtaining and filling out any appropriate paperwork and submitting it to the insurance company for them to reimburse you directly.  I will be glad to fill out any part of the form that is necessary and/or provide a statement that will meet your insurance company's requirements.

 

Payments are to be made at each session, by cash, check or Zelle only please  You will be charged for a missed appointment if you have failed to notify me within 24 hours of our scheduled time.  Fees may change periodically, and thus the fees are subject to change with two weeks prior notification.
